Verify Your Residency or Fellowship Training
We offer Residency and Fellowship training verification services for alumni. To request verification, simply download and complete the Training Verification Invoice (PDF), print and mail it with the required payment.
Each verification request incurs a $130 processing fee, and requests are processed within 10 business days of receipt. If specific form is required by your institution, please include; otherwise, we will issue our standard form.
