• Treatment – We share information with doctors, nurses, and other clinicians directly involved in your care.
• Payment – We submit data to insurers, Medicare, or Medicaid to obtain reimbursement.
• Healthcare Operations – Quality assessment, accreditation, or medical-education activities.

 

3. Other Permitted or Required Disclosures

• Public-health reporting, abuse or neglect, FDA or CDC notifications.
• Law-enforcement requests, subpoenas, or court orders.
• Organ-procurement organizations and research (with IRB approval or your consent).

 

4. Your Rights

• Access or request a copy of your medical record.
• Request corrections to your record.
• Receive an accounting of certain disclosures.
• Request restrictions or confidential communications.
• Receive a paper copy of this notice upon request.
• File a complaint without fear of retaliation by contacting our Privacy Office.
