Training Verification

Residency and Fellowship training verification requests may be submitted to the Education unit of UC San Diego Radiology Administration.

Submit a Request

Training Verification Invoice (PDF): Fill in and print this form. Instructions for mailing the invoice and your check are included on the form.

• Payment: Once we receive the invoice and payment, we will process your request within 10 business days. Due to the high volume of requests for verifications, an administrative processing fee of $130.00 is assessed for each new verification.
• Note: If your institution requires a specific verification form, include a copy with your invoice and payment. Otherwise, we will issue our standard verification form.
